Draft Terms of Use

Not a production contract. This draft demonstrates how terms can be viewed, downloaded, versioned and acknowledged. Philippine counsel, the Data Protection Officer and accountable clinical partners must approve the production text.

1. What CareComm+ is

CareComm+ is a proposed coordination service that helps beneficiaries, caregivers, community partners, sponsors, clinics, laboratories, clinicians, pharmacies and administrators complete an agreed preventive-care journey. CareComm+ does not replace a licensed healthcare professional, responsible health facility, emergency service, regulator, benefit administrator or medical-record system.

2. Account and role

Use your own account and provide accurate, current information.
Access depends on verified identity, role, current authority, assigned purpose and the sensitivity of the action.
A contact check is not professional-credential verification and does not grant clinical access.
Do not share passwords, impersonate another player or use information outside an assigned purpose.

3. Role schedules

The Role Acknowledgement Schedule forms part of this draft. Additional agreements may apply to a partner organization, event, family proxy, payment or clinical integration.

4. Health and emergency limits

Clinical decisions, orders, result validation, explanation, referrals and prescriptions remain with the licensed professional and responsible facility.
Do not use the service for emergencies. Use the appropriate local emergency or clinical channel.
Scheduling, eligibility or benefit information shown by the system must be confirmed by the responsible source where required.

5. Privacy and acceptable use

Use only the minimum information necessary for an authorized task. Unauthorized browsing, disclosure, scraping, marketing, discrimination, automated clinical decision-making or reuse of personal or health data is prohibited. See the Draft Privacy Notice.

6. Acknowledgement and records

The production service should store the document version and cryptographic hash, time, account, role, authority, step-up method, event or purpose and receipt identifier. A general Terms acknowledgement does not replace a separate health-data consent, lawful-basis record, family proxy grant, event acceptance or partner contract.

7. Suspension and correction

Access may be limited or suspended when a credential expires, authority is withdrawn, a partner is blocked, a safeguarding or security concern arises, or use falls outside scope. Users must be able to request correction and follow the applicable review process.

8. Changes

Material changes require a new version and, where appropriate, a new acknowledgement. Previous versions and acceptance evidence must remain auditable according to the approved retention schedule.
